چکیده

An 86-year-old woman with a sacral pressure ulcer who was in a bedridden state due to terminal dementia was admitted complaining of a headache and fever. She showed an impaired consciousness and meningeal irritation without any other neurological deficits. A cerebrospinal fluid examination revealed leukocytosis (2,500/mm, 92% polymorphs) and hypoglycorrhachia. Head computed tomography (CT) findings showed multiple air bubbles in the subarachnoid space and the ventricles (Picture 1). The sacral pressure ulcer contained exposed bone, but the dura mater was not visible. The leakage point could not be detected macroscopically (Picture 2a). Spinal CT revealed the presence of a pneumatocele in the spinal canal. In addition, the terminal of the spinal canal formed a fistula at the sacral region through the thin bone cortex (Picture 2b, arrow). These findings were totally resolved following the treatment, which included antimicrobial administration, of the pressure ulcer. We herein report the first case of bacterial meningitis with pneumocephalus where direct radiological evidence is presented concerning the communication between the external air and the spinal canal through a pressure ulcer (1, 2).

Fatal Meningitis in a 14-Month-Old with Currarino Triad

We report a case of a 14-month-old girl with undiagnosed Currarino triad presenting acutely with meningitis caused by enteric commensals. Head CT demonstrated a large pneumocephalus. A fistulous neurenteric tract through a presacral mass was present on spine MRI and abdominal CT. The patient had a history of constipation for the last three months. However, an underlying diagnosis of Currarino t...

Pneumocephalus associated with Bacteroides fragilis meningitis.

Gas within the intracranial cavity (pneumocephalus) commonly results from trauma or after surgery and rarely from infection by gas-forming organisms. The presence of pneumocephalus in the absence of injury or surgery should raise the suspicion of anaerobic infection of the central nervous system. I present a case of pneumocephalus associated with Bacteroides fragilis meningitis where the diagno...

Spontaneous pneumocephalus associated with open myelomeningocele.

Myelomeningocele is usually complicated with intracranial infections such as meningitis and ventriculitis. Spontaneous pneumocephalus should be kept in mind as a rare complication of open meningomyelocele. Any patient with open meningomyelocele should be carefully evaluated for pneumocephalus with a cranial USG. This information is useful prognostically as well as has therapeutic implications.

Diffuse pneumocephalus from Clostridium perfringens meningitis: CT findings.

Pneumocephalus is defined as "the presence of air or gas within the cranial cavity" [1] , but no distinction is made between localized and diffuse gas collections . We report a case of diffuse pneumocephalus caused by the gas-forming organism Clostridium perfringens. To the best of our knowledge, this is the first demonstration of diffuse pneumocephalus associated with Clostridium meningitis an...

Spontaneous Pneumocephalus Caused by Pneumococcal Meningitis

Pneumocephalus is a condition characterized by the presence of air in the cranium, and it is mainly caused by trauma or a neurosurgical procedure. In the absence of head trauma or a neurosurgical procedure, meningitis is an extremely rare cause of pneumocephalus. Here, the authors present a rare case of spontaneous pneumocephalus caused by pneumococcal meningitis, in which simple lateral radiog...
